Documents Required for Hajj Visa

Completed and signed.
- Original Passport:
Valid for at least 6 months from the travel date (with at least 2 blank pages for visa).
- Non-American Citizens:
Clear copies (front & back) of Permanent Resident Card (Green Card).
- H1 Visa Holder:
Copy of H1 or Work & Re-Entry Permit.
- Photographs:
Four recent passport size photographs with white background and applicant’s name on the back of each photo.
- Vaccination:
Original vaccination certificate for meningitis (valid up to 3 years). Not required for children under 12 years.
- Marriage Certificate:
Copy of notarized marriage certificate for husband & wife.
- Birth Certificate for Children:
Copy of birth certificate (if last name on the passport is different).
- Children traveling with one Parent:
NOTARIZED permission letter from the parent that’s not traveling, with a photocopy of his/her passport and driver license.
- Muslim with a non-Islamic name on the passport: (click here to download)
Statement in writing from the local Islamic center validating their conversion to Islam.
- Sisters accompanied by their Mahram (non-marriageable male relationship such as father, brother, maternal uncle, son etc):
Copy of their birth certificate indicating relationship.
- Sisters traveling alone (must be over the age of 45): (click here to download)
NOTARIZED letter from a mahram giving permission to travel for Hajj with Sirat Al-Mustaqeem Hajj group
(If the last name of the person giving permission letter is different than the applicant then birth certificate indicating the relationship is required)
